“There is this notion that [integrative medicine] is outside of our mainstream biomedical model and that there is not enough time to even teach what we need to teach. But really it depends on how we define good medicine.” Exclusive interview done by the Center of Excellence at Stanford University with Pfeiffer Visiting Scholar, Ka-Kit Hui, MD, FACP.

With a career spanning four decades at the UCLA David Geffen School of Medicine and the UCLA Center for East-West Medicine, Dr. Ka-Kit Hui shares his background and thoughts on how he views integrative medicine, which does not focus on different modalities but rather goes back to the basics of looking at the person in their environment and using healing traditions that work to re-regulate and replete the body’s own system. He emphasizes the importance of having a flexible health model to help patients at different stages of their journey and building a trusting and caring patient-provider relationship. Dr. Hui also briefly addresses medical education and how to have a more comprehensive approach towards patient-centered medicine to optimize the healthcare system. With these insights, Dr. Hui provides us with a new perspective of looking at a patient in a systemic way, as well as a new way of interpreting the information that will benefit individuals and populations.

Introduction:

Stressed? Your teeth may show it. When you are stressed, your muscles tend to tighten; the same concept applies to your jaw muscles and can result in teeth grinding and/or clenching in some individuals. While grinding and clenching can occur subconsciously, there are many self-help techniques one can embrace to decrease these detrimental practices. In this article, we provide some suggested exercises for stretching the jaw muscles and reducing tension in the jaw and also, some further resources to explore to learn more about the impact of stress on oral health.

Self-Help Guide to Reduce Grinding and Clenching:

While mouth guards are the typical prescription given to those suffering from bruxism (problems with grinding/clenching of the teeth), there are many modalities inspired from integrative medicine that can be utilized in combination with or as an alternative to the use of mouth guards.

Jaw Exercises/Mindful Teeth Placement:

Open the mouth as wide as comfortably possible and touch tongue to front teeth. This action relaxes the jaw muscles.

Do not allow your top and bottom teeth to make contact except when chewing. To prevent the teeth from touching, say the letter “n” to position the tongue and keep your mouth in this position as you work to avoid clenching.

Self-Massage:

Massaging your jaw can help your jaw muscles relax and reduce the tension in this area. Additionally, holding a warm towel against your jaw can also promote muscle relaxation.

Personalized Nutrition:

Personalized nutrition involves tailoring a diet specific to your own particular needs. Individuals suffering from bruxism may benefit from decreasing the intake of caffeine and alcohol, which are substances that can augment grinding and clenching problems. Also, consuming mainly soft foods and avoiding tough/hard food items and gum chewing can help prevent overuse of your jaw muscles.

Relaxation Techniques:

For many, clenching and grinding are directly related to struggles with properly managing stress. Utilizing methods to reduce stress can be an effective way to combat bruxism. Many relaxation techniques exist, such as meditation, yoga, Tai Chi, deep breathing exercises, massage, progressive muscle relaxation, and much more. Learn more about these relaxation techniques in Additional Articles to Explore and Related Content below.

This summary covers a book chapter published in the text Complementary and Integrative Therapies for Psychiatric Disorders. The chapter was written by Dr. Ryan Abbott, MD, JD, MTOM, an associate professor at Southwestern Law School, and Dr. Helen Lavretsky, MD, MS, a Professor In-Residence in the Department of Psychiatry at UCLA.

While pharmaceutical medicines have revolutionized the treatment of serious mental disorders, they are ineffective or only partially effective in a significant number of cases, and they can also cause serious side effects. By contrast, mind-body practices tend to be less costly and cause fewer side effects, and they can enhance the effectiveness of prescription drugs or reduce the necessary doses. Identifying safe and effective mind-body practices is therefore important to providing quality mental health care.

To that aim, the chapter by Abbott and Lavretsky summarizes the evidence-base supporting the use of Tai Chi and Qigong in treating and preventing psychiatric disorders. Tai Chi and Qigong are traditional Chinese exercises that are widely practiced for their health benefits and as martial arts. The article notes that evidence suggests that these practices may be effective at treating a range of physical health conditions, and at improving health-related quality of life. Furthermore, that evidence from randomized controlled trials suggests that Tai Chi and Qigong may be effective in reducing depressive symptoms, stress, anxiety, and mood disturbances. Minimal research supports Tai Chi and Qigong as promising treatments for Parkinson disease, traumatic brain injury, insomnia, substance abuse, and cognitive impairment. The authors state that indications for Tai Chi and Qigong include inadequate response to other evidence-based treatments, physical comorbidities known to be responsive to Tai Chi and Qigong, patients interested in exercise-based or mindfulness-based interventions, and in geriatric patients who are more susceptible to adverse effects from pharmacologic therapies.

A new study by Dr. Ladan Eshkevari and colleagues at the Georgetown University School of Nursing uses electronic acupuncture on rats to further understand the molecular basis behind acupuncture's benefit.

According to Dr. Ladan Eshkevari, Associate Professor at Georgetown University School of Nursing and Health Studies, “Many practitioners of acupuncture have observed that this ancient practice can reduce stress in their patients, but there is a lack of biological proof of how or why this happens.” In a new study published in the Journal of Endocrinology, Dr. Eshkevari and his colleagues aimed to explore the biological mechanisms involved in acupuncture’s stress relieving abilities at a molecular level.

For the study, Dr. Eshkevari and colleagues designed a series of tests that used exact same dose of pressure of electronic acupuncture of the point “Zusanli” in rats. Stimulation of this point, which is the same in rats and humans, is reported to alleviate stress and other conditions.

The researchers split the rats into four groups for a 10 day experiment: control with no added stress and no acupuncture, stressed for one hour each day and no acupuncture, stressed for an hour each day and receiving “sham” acupuncture by their tails; and stressed for an hour each day and received genuine acupuncture at “Zusanli” point. Hormones levels were measured as it was released into the bloodstream during the stress reaction from the hypothalamus, pituitary gland, and adrenal gland (together known as the HPA axis), as well as a peptide involved in the “fight or flight” response, called NPY.

Through this study, they discovered that “electronic acupuncture blocks the chronic, stress-induced elevations of the HPA axis hormones and the sympathetic NPY pathway.” These findings, which support acupuncture as a useful tool in combating stress, are encouraging for the improvement of healthcare through the integration of effective Eastern medical practices.

The fourth International Research Congress on Integrative Medicine and Health (IRCIMH) will take place at the Hyatt Regency in Miami, Florida, USA, on May 13-16, 2014. The Congress is sponsored by the Consortium of Academic Health Centers for Integrative Medicine, a group of 54 esteemed academic health centers and affiliated institutions. This Congress will also serve as the 9th annual International Congress on Complementary Medicine Research (ICCMR) sponsored by the International Society for Complementary Medicine Research (ISCMR). The Consortium is committed to making this Congress the premier international forum for Integrative Medicine research.

Integrative Medicine is the practice of medicine that reaffirms the importance of the relationship between practitioner and patient, focuses on the whole person, and is informed by evidence. Integrative medicine includes many disciplines, types of practitioners, and therapeutic approaches; the evidence base is complex and growing quickly. The International Research Congress on Integrative Medicine and Health focuses on the evidence base in the practice of Integrative Medicine. We expect more than 1,200 researchers, clinicians, and trainees from around the world to attend. The Congress organizers invite individuals from all disciplines and professions engaged in integrative medicine and health to attend.

The IRCIMH’s mission is to foster the development of new collaborations and to strengthen existing partnerships within the field of Integrative Medicine. This Congress will showcase original scientific research through keynote and plenary sessions, oral and poster presentations, and innovative sessions. Integrative medicine and health research areas to be presented and discussed include basic science, clinical trials, lifestyle and prevention, methodology, health services, cost effectiveness, and education.

The 2012 International Research Congress on Integrative Medicine and Health was held in Portland, Oregon. The Congress had over 1000 participants, representing 30 countries. Of the attendees, over 250 were trainees, students, and new investigators. We saw an increase of over 200 attendees from 2009 and we anticipate the Congress in Miami to continue to grow in its international stature and number of participants.

The explosive growth of Complementary and Integrative Medicine has brought both opportunities and challenges to patients and consumers. Patients are confronted with a bewildering and growing array of promises and claims. This presents a unique challenge to physicians and other health care professionals tasked with helping their patients make informed health care decisions. Researchers and clinicians from Mayo Clinic offer this course to provide learners with a general working knowledge of Complementary and Integrative Medicine; familiarity with some of the most common therapies; and most importantly, practical application of these therapies to patients with relevant diseases and conditions. Mayo Clinic, a member of the Consortium of Academic Health Centers for Integrative Medicine, has been actively engaged in research and is a leader in applying such information to clinical practice.

Learning Objectives

Upon conclusion of this program, participants should be able to:

Discuss historical and current use of Complementary and Integrative Medicine practices/therapies by patients in the U.S.

Synthesize existing scientific evidence regarding safe and practical applications of Complementary and Integrative Medicine therapies for integration into conventional care.

Discuss underlying principles to promote communication with patients (and their families) regarding issues and challenges related to the use of Complementary and Integrative Medicine modalities.

Reflect on Complementary and Integrative Medicine and their clinical practice.

Attendance at this Mayo course does not indicate nor guarantee competence or proficiency in the performance of any procedures which may be discussed or taught in this course.

The course intends to provide participants with awareness and basic knowledge of Complementary and Integrative medicine; familiarity with some of the most common therapies; and most importantly, practical application of these therapies to patients with relevant diseases and conditions. This course has been customized towards practicing health care practitioners (physicians, physician assistants, nurse practitioners, nurses) who work with patients using or interested in using complementary and integrative medicine therapies. Residents, medical students, nurses and other members of the health care team may also benefit from this program.
